Abstract: 

Doppler ultrasound system is of great importance among the diagnostic modalities. To ensure the output of such systems that measure the physiological quantities, performance tests were carried out using AIUM and BSI protocols on 20 pulsed wave Doppler systems in 9 reference centers in Tehran. In these tests, velocity of Doppler ultrasound systems as an acoustic parameter was assessed. To evaluate the performance of pulsed wave Doppler, an in-house string phantom was designed for the first time in the country. This phantom was used to investigate pulsed Doppler ultrasound systems. The results show that the minimum and maximum errors in blood flow velocity measurement on 20 Doppler systems were 23 (cm/s) and 83 (cm/s), respectively. The mean± SD for velocity measurement errors in 6 Doppler systems (7. 5 MHz) and 12 systems (3. 5 MHz) were 11± 16 and 28± 9 (cm/s), respectively, the velocities ranged 10-140 (cm/s). These errors emphasize the necessity for doing performance tests in Doppler ultrasonic systems.

Abstract: 

Background: Lichen planus is a chronic inflammatory disease associated with an increased risk of subclinical atherosclerosis and cardiovascular diseases. This study aimed to evaluate patients with lichen planus using carotid Doppler ultrasound parameters. Methods: Forty patients with lichen planus and 40 controls were included in this study. Common carotid artery intima-media thickness (CIMT) and the number of atherosclerotic plaque were measured and compared to the control group. Total cholesterol, triglyceride, low-density lipoprotein (LDL) and high-density lipoprotein (HDL) levels were measured. Leptin level (Pg/ml) was measured using the enzyme-linked immunosorbent assay (ELISA) method (Leptin ELISA kit, Orgenium, Finland). Results: Significant difference was found between the groups in terms of CIMT (P=0. 005). The median range for blood leptin level, triglyceride, cholesterol, and LDL was higher for lichen planus patients than for controls. We found a significant difference between the severity of LP and CIMT (P=0. 035). No statistical difference was found between LP and the number of atherosclerotic plaque. Conclusions: Our study suggested that measurement of the mean intima media wall thickness of the common carotid artery could be beneficial as a valuable method for early diagnosis of atherosclerosis in lichen planus.

Abstract: 

Purpose: To evaluate the role of Doppler ultrasonography (DUS) in diagnosing pediatric testicular torsion (TT), and its diagnostic accuracy, and helping clinicians increase specificity and decrease negative exploration rates.Materials and Methods: We performed a retrospective study of all consecutive patients with acute testicular symptoms referring to our pediatric emergency department (ED) from January 2010 to December 2013.Results: We analyzed 1091 patients, with a mean age of 9 years. DUS was performed in 498 patients (40.8%); 107 patients (8.8%) underwent surgery and 41 patients (3.3%) had a TT. The following clinical findings were collected: presence of scrotal pain, erythema and swelling, spermatic cord pain and abnormal cremasteric reflex. The clinical findings significantly associated with TT were spermatic cord pain (OR=37, 95% CI: 11.9-111.1, P<.001) and abnormal cremasteric reflex (OR=47.6, 95% CI: 13.5-166.6, P<.001); the presence of swelling was not associated with TT (OR=2.3, 95% CI: 0.7-8.4, P<.001). Scrotal pain was not significantly associated with TT (P=.9), while erythema made TT unlikely (OR=0.22, 95% CI: 0.07-0.7, P=.0445). In all cases the DUS significantly increased the predictivity.Conclusion: TT was present in 3.3% of patients presenting with testicular symptoms. The predictivity based on clinical findings resulted high and the negative exploration rate for TT was 62%. DUS increased the predictivity in all patients.

Abstract: 

Background: Effective cardiopulmonary resuscitation (CPR) is critical for survival after cardiac arrest. Traditional methods of assessing CPR effectiveness, such as pulse palpation, have limitations. This study investigated the utility of Doppler ultrasound and echocardiography for real-time hemodynamic assessment during CPR. Objectives: The objectives of this research were to evaluate the effectiveness of CPR using Doppler ultrasound and echocardiography and to correlate these findings with patient demographics, presenting symptoms, medical history, and resuscitation outcomes. Methods: This one-year descriptive study enrolled 100 adult patients requiring CPR in the emergency department of Be'sat Nahaja Hospital. Data collected included patient demographics, presenting complaints, medical history, vital signs, and hemodynamic parameters [peak systolic velocity (PSV), pulse peak point, and ejection fraction (EF)] obtained via Doppler ultrasound and echocardiography during CPR. Statistical analysis, including repeated measures two-way ANOVA, was performed to assess the relationship between these variables and resuscitation outcomes [return of spontaneous circulation (ROSC) and survival to hospital admission]. Results: The mean age of patients was 63.46 years, with 68% being male and 70% experiencing out-of-hospital cardiac arrests. Return of spontaneous circulation was achieved in 22% of cases, and survival to hospital admission was 17%. Hemodynamic parameters, including PSV, pulse peak point, and EF, varied significantly across CPR phases. Successful cases showed higher values during active cardiac massage (e.g., mean PSV of 75.3 cm/s in successful cases vs. 11.8 cm/s in unsuccessful cases). A four-phase classification of cardiac activity was proposed based on these parameters, ranging from "No Effective Cardiac Activity" (PSV/pulse peak point/EF ≤ 10 cm/s) to "Effective Cardiac Activity" (> 40 cm/s). Conclusions: Doppler ultrasound and echocardiography provide valuable real-time hemodynamic assessment during CPR, offering objective measures of cardiac function and blood flow that correlate with resuscitation outcomes. These technologies can aid in the evaluation of CPR effectiveness.

Abstract: 

Background: To explore the application of contrast-enhanced ultrasound and color Doppler ultrasonography in the treatment of primary hepatocellular carcinoma (PHC) after Transhepatic Artery Chemoembolization (TACE). Materials and Methods: 49 patients with primary hepatocellular carcinoma were treated with TACE for 3-5 times. Before and after treatment, digital subtraction angiography, contrast-enhanced ultrasound and color Doppler ultrasound were performed. The lesions before and after contrast-enhanced ultrasonography and color Doppler ultrasonography were compared. The residual or recurrence and swelling of lesions before and after contrast-enhanced ultrasonography and color Doppler ultrasonography were analyzed. Detection of the capsule of the lesion. Results: The detection accuracy of contrastenhanced ultrasound (95. 77%) was significantly higher than that of color Doppler ultrasound (84. 51%) (P<0. 05). The detection rate of tumor envelope by contrastenhanced ultrasound (22. 54%) was significantly higher than that by color Doppler ultrasound (9. 86%) (P < 0. 05). The accuracy of detecting residual or recurrent lesions by contrast-enhanced ultrasound (91. 67%) was significantly higher than that by color Doppler ultrasound (58. 33%) (P<0. 05). Conclusion: The rate of bone metabolism of perimenopausal women in Xuzhou is not optimistic. We need to pay close attention to the risk of bone metabolism disorder of perimenopausal women with older age, diabetes mellitus and osteoporosis, so as to provide useful help for improving the quality of life of perimenopausal women in Xuzhou.
